HEALTH CONCERNS
Please note: Unless you indicate otherwise, information given to the Health Office will be shared on a 'Need to Know' basis where the safety and welfare of you child is at stake. Only relevant information will be shared such as emergency contact information, allergies, and medical issues that could possibly manifest themselves while your child is not in close proximity to the nurse. Only teachers and staff that would be in a supervisory capacity over your child would be authorized to access that information.
STATE MANDATED PHYSICALS
Health Appraisals are required for all new entrants and for children in grades PreK-4,, K, 2, 4, 7, and 10. These exams should be done by your private healthcare provider. This is also an excellent time to make sure your child has a complete dental examination by your private dentist. Please call me if you are unsure of the status of your child’s physical form.
FOR ALL NEW ENTRANTS
Immunization records, completed by your health care provider are required for entrance into school.
ABSENCES
If your child will be absent from school for any reason, please call and notify the school. If you don’t call or send a note, I will call you. When your child returns to school, please have your child bring a written note to his / her teacher explaining the reason for the absence. If you are aware that your child will be absent for an appointment (doctor, dentist, etc.) please send that note in before the appointment.
CONTACT INFORMATION
Please notify the school should your phone number (home, cell, and work), emergency contact phone number, or your contact person change. In the event that your child is ill, I need to be able to contact you or your designated contact person.
CONTAGIOUS DISEASES
Please notify the health office of any contagious conditions such as strep, conjunctivitis, or head lice.
MEDICATION
All medication must be brought to school in the original container by a parent or guardian. All medication, including over the counter medication, must be accompanied by a doctor’s order and written parental permission.
IMMUNIZATION LAW
All children born on or after January 1, 1994 and entering the sixth grade on or after January 1, 2005, must demonstrte proof of immunity to Varicella (chicken pox). Proof is either: 1) physician documented disease, 2) serological evidence by a blood test, or 3) certificate of immunization. Nothing else is acceptable. The school must exclude anyone who does not have the proper proof on the first day of school in September 2005. It is essential that you call your health care provider now to schedule the vaccine if your child has not already completed the immunization or had the disease documented by a physician. Failure to present proof of immunization on or before the first day of sixth grade will, of necessity, result in your child being turned away from school at the door. Please call for an appointment today and send proof of immunization to the school nurse as soon as the shot is completed. Thank you for your prompt attention to this important matter
NEW IMMUNIZATION LAW
Starting September 1, 2007, all students born on or after 1/1/94 AND entering sixth grade on or after September 1, 2007 are required to have a booster shot of Tdap BEFORE the start of the school year.
